1. Assumption of Risk

I understand that participation in a basketball program involves physical activity and may include risks such as falls, collisions, or other injuries. I acknowledge that these risks cannot be completely eliminated even with proper supervision.

2. Release of Liability

By signing this form, I release and hold harmless Greenhill Islamic School, the PTC, program organizers, volunteers, and coaches from any liability, claims, or demands arising from my child’s participation in this program, except in cases of gross negligence.

3. Medical Consent

In the event of an injury or medical emergency, I authorize the program organizers or supervising adults to seek appropriate medical care for my child. I understand that I am responsible for any medical costs.

4. Supervision & Conduct

I understand that:

- Children must follow instructions from coaches and supervisors.

- Unsafe behaviour may result in removal from the session for the safety of all participants.

- A responsible adult will be supervising the gym at all times.

5. Acknowledgment

I have read and understood this waiver. I agree to the terms and allow my child to participate in the 6‑week basketball program.

6. Photography Permission (Optional)

I give permission for my child to be included in group photos or videos taken during the program for school/PTC updates and community sharing.
